Omissions

On page 32, I gave Kelland's dissection of a gnomon to a square, but didn't include it in the Index of Dissections.

On page 37, I mentioned Sam Loyd's dissection of squares for 1² + (sqrt(8))² = 3², noting that it is lurking in Fig. 4.24. I should have included it in the Index of Dissections.

The dissection of a {14/3} to two {7/2}s was not included in the Index of Dissections. The dissection appears on page 180.

On page 270, I gave Dudeney's dissection of a mitre to a square, but failed to include it in the Index of Dissections.

Other errors in the Index of Dissections

Under {3}:

    1² +2² +2² = 3², 45
        should be preceded by a "for".

    2² +3² +(sqrt(12))² = 5², 45
        should be preceded by a "for".

    for x² +y² +z² = w²
        should have both entries together and should precede
            for x² +y² +z² + w² = v².

Under {4}:

    {tilde 3}, 136
        should be preceded by a "for".

    2² +3² +(sqrt(3))² = 4², 37
        should be preceded by a "for".

Under {L}:

    {12/5}, 211
        should be 21 rather than 211.
